GLOSSARY OF TERMS  
This defines terms commonly used in the FASFA lending process.

NON-TAX FILER STATEMENT (2011-2012)
Students should complete this form to explain sources of income if a federal income tax statement was not filed. 

PLUS DIRECT LOAN - ACCEPTANCE FORM
The parent of the IUON Student uses this form to accept, decline, or reduce the amount of your PLUS loan. 

PLUS DIRECT LOAN - AUTHORIZATION FORM
The parent of the IUON Student authorizes IUON to use the PLUS finanncial aid funds to pay for other educational related expenses that may be charged to the student’s account including books, student and dependent insurance, and penalties/fines and other incidental educationally related charges.

ENTRANCE AND EXIT COUNSELING

SIBLING VERIFICATION (2011-2012)
The parent of the IUON Student completes this form if the IUON student has other siblings in college next year.

STUDENT DIRECT LOAN - ACCEPTANCE FORM
This form is used to accept, decline, or reduce the amount of your Direct Subsidized and/or Direct Unsubsidized loan.

STUDENT AUTHORIZATION FORM
The student authorizes IUON to use the loan funds to pay for other educational related expenses that may be charged to the student's account including books, student and dependent insurance, and penalties/fines and other incidental educationally related charges.

STUDENT NCLEX AUTHORIZATION FORM 
Foreign nursing schools must obtain all results of the NCLEX-RN achieved by students and graduates who are U.S. citizens, nationals, or eligible permanent residents, together with the dates the student has taken the examination (including any failed examinations) and provide the results to the Department annually.

YOUR OTHER PARENT (2011-2012)
The student completes this form if there is a “noncustodial parent” because IUON requires personal and financial information from the “noncustodial parent” as part of the financial aid process.
